If your doctor enrolled you in a CCM (Chronic Care Management) program, he or she will have only the access to your Diary which you have granted them, as a Guest connection. However, many doctors lack time even to get through their own records during a consultation, so whether your doctor uses your Diary cannot be guaranteed. Secondly, it is important to keep in mind that The Diary is not clinical software and therefore not an official record of the doctor’s clinical decisions.
Many doctors are likely to want to look at a printed version of your Diary Timeline, which is a view of your health information – all on one page. This gives your doctor valuable background about your health story that they will never see via their own software. Your Diary gives a much more complete picture about you than their systems are able to capture.
If you take to your doctor printed pages from your Timeline, we recommend that you print a multi-year view for the big picture, and a 12-month view for current trends, as well as the past several weeks for the latest information.
