Quality measures are tools that help measure or quantify healthcare processes, outcomes, patient perceptions, and organizational structure and/or systems that are associated with the ability to provide high-quality health care and/or that relate to one or more quality goals for health care. These goals include: effectiveness, safety, efficiency, being patient-centered, equitability, and timely care.
Quality measures in The Diary may be predefined patient tasks that need to be completed at a certain time interval. They are selectable when editing a Care Plan for a patient. For example, someone with a risk of breast cancer may be scheduled for the Quality Measure "mammogram", annually.
Quality measures must be set in the CarePro web app, before being assigned to a patient for completion via the CarePro iOS app. To set up a Quality Measure, log in to your Diary web account at thediary.com
Next, go to System Management, on the left side of your CarePro dashboard.
Click on "Quality Measures" at top, then on "New Item" at right.
You will see a "Create Quality Measure" screen. Fill it out with the appropriate information by typing in each box. The following is an example:
Remember to click "Save, once you have completed all fields. You can now assign the Quality Measure to a patient via your CarePro iOS app. NOTE: a preset Quality Measure can only be assigned to a patient with insurance that matches the "Insurance Payer" field in the "Create Quality Measure" screen.
